Verification of Training

To verify a person has completed a residency or fellowship in Yale Radiology & Biomedical Imaging, please email ydrverifications@yale.edu.

To have your verification forms completed with a signature:

There is a fee of $77.00 US funds for processing detailed program verification and forms.

Please submit your verification forms / packet along with signed consent and provide verification fees.
